THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Responsibilities

Cavier Therapy is required by law to:

  • Maintain the privacy and security of your protected health information (PHI)

  • Provide you with this Notice of our legal duties and privacy practices

  • Follow the terms of this Notice currently in effect

  • Notify you if a breach occurs that may have compromised the privacy or security of your information

How We May Use and Share Your Health Information

We may use and share your PHI in the following ways:

Treatment

We may use and share your PHI to provide, coordinate, or manage your care and related services.

Payment

We may use and share your PHI to bill and receive payment for services. This may include working with insurance plans and billing support services.

Health Care Operations

We may use and share your PHI to support practice operations, such as quality improvement, administration, and activities needed to run the practice.

As Required or Permitted by Law

We may share PHI as permitted or required by law, including in situations related to:

  • Public health and safety

  • Health oversight activities

  • Court orders, subpoenas, or other legal processes

  • Reporting abuse, neglect, or domestic violence, when required by law

  • Preventing or reducing a serious and imminent threat to health or safety

Uses and Sharing That Require Your Written Authorization

We will obtain your written authorization before using or sharing your PHI for purposes not described in this Notice, unless otherwise permitted by law.

You may revoke your authorization in writing at any time. If you revoke it, we will stop using or sharing your PHI for the reasons covered by your authorization, except where we have already taken action.

Your Rights

When it comes to your PHI, you have certain rights.

Get a copy of your records

You can ask to see or get an electronic or paper copy of your health information.

Ask us to correct your records

You can ask us to correct health information you believe is incorrect or incomplete.

Request confidential communications

You can ask us to contact you in a specific way (for example, only by email or only at a specific phone number).

Ask us to limit what we use or share

You can ask us not to use or share certain health information for treatment, payment, or operations. We are not required to agree to every request, and we may say no if it would affect your care.

Get a list of certain disclosures

You can ask for a list (accounting) of certain disclosures of your PHI, as allowed by law.

Get a copy of this Notice

You can ask for a paper copy of this Notice at any time, even if you agreed to receive it electronically.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Cavier Therapy and/or with the U.S. Department of Health and Human Services.

We will not retaliate against you for filing a complaint.

Third-Party Services

Cavier Therapy may use third-party services to support scheduling, telehealth, and billing. These services may receive or process PHI as needed to support your care, payment, or operations, in accordance with applicable requirements.
